Questions and Answers - Adult Social Care Portal



Question: Do I have to register for an Adult Social Care Portal Account?

Answer: You can use the portal for information, advice and signposting. You do not need to register unless you want to submit a personal referral to Adult Social Care.



Question: Can I refer more than one person using the Adult Social Care Personal Referral?

Answer: Yes, you can refer as many people as necessary to Adult Social Care however each person must have their own individual Personal Referral (you can not refer more than one person per personal referral/form).



Question: How can I see the Personal Referral I have submitted?

Answer: When you have submitted a personal referral, you will be able to view it under "Submitted Forms". You can access this by clicking your name in the top right-hand corner of the screen and selecting "Submitted Forms" from the drop-down menu. It is recommended that you print or save a copy of any referral submitted to Adult Social Care, as they will only be available in your portal account for a period of 180 days.



Question: How do I continue with a referral I previously saved for later?

Answer: When you save a referral to complete later, you can access it under "Recover a Form". You can access this by clicking your name in the top right-hand corner of the screen and selecting "Recover a Form" from the drop-down menu. This will display any forms you have previously saved and, upon selecting the hyperlinked, red text, the form will open for further editing. These will be available in your portal account for a period of 180 days.



Question: I forgot to add something to my personal referral. Can I go back into it and add more detail once I've submitted it?

Answer: Once you have submitted your referral, you won't be able to amend it. If your circumstances have changed, you can send another personal referral via the Adult Social Care Portal or if urgent contact Family Connect on 01952 385385 option 3.
